New article: A scoping review of Methods, Guidelinelines, and Protocols used for Dose Reduction of Discontinuation of Antipsychotic Medication

This scoping review investigates the various methods used to reduce or discontinue antipsychotic medications in individuals with psychotic disorders, addressing a significant gap in clinical guidance. It analyzed 32 studies—28 focused on oral antipsychotics and 4 on long-acting injectables—revealing three main tapering approaches: immediate, rapid (under 4 weeks), and gradual (ranging from 4 to over 26 weeks). Strategies varied by whether dose reductions were based on the original starting dose or the most recent dose, with some studies employing a hyperbolic tapering method aligned with dopamine receptor activity. Despite the diversity of approaches, no consensus emerged on the safest or most effective method. The findings aim to inform future research and support the development of evidence-based guidelines for antipsychotic discontinuation.
